What does Solara offer?

Solara Care Solutions offers Private Care, Home Health, and Outpatient Therapy. Our coordinated range of services support recovery, promote ongoing wellness, and enhance daily living.

How are Solara Care Solutions and Fleet Landing connected?

Solara Care Solutions brings together Fleet Landing Home Health, Outpatient Therapy, and Private Care under one name and structure. While the name is new, our foundation is the same. We are the same trusted team, delivering exceptional services while achieving quality outcomes and satisfaction.

Who can receive care from Solara?

Solara serves adults living within Jacksonville and its Beaches, whether at home or in a senior living community. Personalized care is open to anyone who would benefit from Private Care, Home Health, and Outpatient Therapy.

How can Solara help me?

Solara can support you with everyday needs at home, such as getting ready for the day, meal preparation, errands, or accompaniment to appointments. We also provide nursing and therapy services following a hospital stay, planned surgery, or medical event—either in your home or in our Outpatient Therapy gym.

Do I need a physician referral to get started?

For Medicare-certified Home Health and Outpatient Therapy, a physician referral is required. Private Care services do not require a referral, and families can contact us directly to discuss needs or schedule support. What happens after I request a care consultation?

After your initial inquiry, a member of our team will connect with you to understand needs, answer questions, and explain next steps. From there, we’ll coordinate any required assessments, referrals, or scheduling to ensure care begins smoothly and promptly.

What do services cost?

Costs vary based on your needs and goals, Solara Care Solutions may be paid for privately, covered by your long-term care insurance, or accessed as a benefit of your health insurance. Our team will guide you based on your individualized plan of care, so you know what to expect.

How quickly can services begin?

Home Health services and most Private Care start within 48 hours, depending on staff availability and the specifics of your needs. Outpatient therapy evaluations can typically be scheduled promptly as well. Our team will walk you through next steps and timelines during your initial consultation.

Can services change over time if needs change?

Yes. Solara’s care plans are designed to adapt as needs evolve. Whether care needs increase, decrease, or transition between services, our team works closely with individuals, families, and providers to ensure continuity and appropriate support.

Where does care take place?

Care can be provided where you need it most. Whether at home, in the hospital, or at our Outpatient Therapy gym located in Fleet Landing Atlantic Beach, our team is here for you.

How are families involved in the care process?

Families are an important part of the care journey. With consent, Solara keeps families informed, involved, and supported through regular communication and coordination with the care team.

Does Solara coordinate care with physicians or facilities?

Yes. Solara works closely with physicians, hospitals, rehabilitation facilities, and senior living communities to ensure care is coordinated, aligned, and responsive throughout the care journey.

What types of insurance does Solara accept?

Solara works with Medicare and many major insurance plans for eligible Home Health and Outpatient Therapy services. Private Care is private pay or covered by your long term care insurance. Because coverage can vary by plan and service, our team is always happy to walk through your benefits and help you understand your options.
